What is Heart-To-Heart?
|
| Heart-To-Heart is an “adult day program”. People come from their home or apartments to spend the day and return back home in the afternoon.
Time is spent with people you already know and making new friends. The day goes by fast as there are a variety of things to do: coffee and snacks two times a day, a good noon meal, sharing current events from the newspaper, going for walks, exercises. Learning more about travel and health topics, reminiscing, cooking and baking, playing different games (bowling, parachute, trivia, cards). Oh yes, there’s time to rest on our chairs and sofa.

| A day-service program for persons who need extra support to remain in their community. We provide services specifically designed to meet the increasing needs of adults. |

| Nutrition |
Our clients get a good balanced meal when they are here. There is a dietician on staff that will work with you if you have a special diet. If you don’t like what is served on a particular day you will be served something different. You can get a regular portion or a small portion. Two snacks are served per day-one in the morning and one in the afternoon. On birthdays you will receive a special treat!

| You may provide your own, or contact the Renville County Public Transportation System at (320) 523-3589 for transit arrangements. |
| What is covered? |
| Time sees all types of changes. One is that persons who are on “home bound status” to receive Medicare benefits for a specific purpose, are now able to attend Heart-To-Heart and not jeopardize their Medicare status benefits. Heart-To-Heart is not Medicare certified for any of its services. More insurances are also seeing the benefits of adult day services. But, you need to check with your specific insurance representative. Many long-term care policies also include our program for coverage. The important thing to remember is you need to ask specifically what is your coverage, how much is covered and any other questions you might have. |
