Advance Directives for Health Care

At any age, a medical crisis can leave anyone unable to make their own medical decisions.
Advanced care planning provides information to others to care for you in the manner you choose.   
The Advanced Care Planning Process includes:
• Clarifying your health care goals.
• Understanding treatment options
• Weighing your options based on your values and they type of care and treatments available.
• Decision making about who you want to appoint as your health care representative.
• Communicating your wishes in writing, which is called a health care directive
